Buckinghamshire CCG COVID19 Response - adding the eFi score to all patients over 65 years old
As part of Buckinghamshire CCG's response to the COVID19 Pandemic, frailty assessments are being carried out and patients at risk are being scored using the Rockwood Clinical Frailty Score.
The 'at risk' patients whose records need manual review (and the Rockwood score calculating) are being defined according to a variety of parameters, one of which is having an eFI (electronic Frailty Index) score indicating possible moderate or severe frailty.
These instructions explain how to do batch add an eFi score to all patient records. Some organisations, including EMIS Health and the BMA, have in the past not endorsed doing this. This is due to concerns about clinicians misunderstanding what an eFi score is.
How to batch-add the eFi score to all patient records

- An eFI score does NOT mean a patient is frail. It is simply a calculation based on data within their clinical record on the day the eFi score is calculated indicating their risk of frailty.
- The eFi score should be used as a guide to identify patients who may be suffering from frailty.
- Buckinghamshire CCG is asking practices to record a Rockwood Clinical Frailty Score on those patients with an eFi score that indicates possible moderate/severe frailty.
How to batch-add the eFi score to all patient records
- Please confirm with the Partners that they are happy for this process to occur.
- If you are using Resource Publisher then you need to copy the protocol out of the Frailty folder in the the Ardens "Shared Folder" to a local folder first (please search for "add efi score to record" to find this). If you can't see it please e-mail [email protected].
- Run the "Patients eligible for frailty score" search, found in the Ardens-QMasters search folders here:
- This will find all patients over 65.
- Go to Batch data manager by selecting 'Reporting --> Batch Data Manager' from the EMIS main menu.
- Select 'Add -> Protocol' from the top menu.
- In the 'Batch add' field, search for our "add eFi score to record" protocol.
- In the "Description field" write "add eFi score to over 65s".
- Under 'List type', select "search population (included)"
- In the 'Search population (included)' field search for the "Patients eligible for frailty score" search in our folder.
- The number of patients it will be added to will be displayed (this is 0 in our dummy system) - see below
- An example of how it should look is below. Once you're happy, click "OK". The eFI calculation will be added to all the records; this can take an hour or so.
- Now you can run the searches we have provided to help you plan how you will manage frailty in your GP practice.
