DEM004 Dementia review coding for QOF

There have been some queries from practices around the DEM004 indicator and the required codes for inclusion.

Patients who have had a Dementia Care Plan agreed or reviewed at a face-to-face encounter in the year should be included in the DEM004 'included' population. However, patients who have been coded with 8CMZ - Dementia care plan or 8CSA - Dementia advance care plan agreed will currently be showing as 'excluded' according the v39 searches in EMIS Web.

This has been queried with EMIS Health, who replied as follows:

"...the code 8CSA was not included in the version 39 business rules. It has been re added by NHS Digital in the version 41 business rules which should be published and added into EMIS shortly (we have not been provided with an exact date as of yet.)"

​The draft version of the v41 rules also includes the SNOMED-CT code 1095121000000102 which maps to the READ code 8CSA. Click here to view the draft v41 business rules.

Therefore, it appears you can continue to record these codes as dementia care plans but until your practice is updated  with the v41 business rules searches, these patients will mistakenly show as 'excluded' in your Population Reporting dashboard.
